Outpatient Services

New Patients

After filling in the required information on the Application Form, submit it together with your Health Insurance Card and letter of referral to the Reception for First Time Visitors (#1).
You will receive your Hospital ID Card and a registration slip in a plastic file. Please submit it to the proper department receptionist.

Returning Patients

If you have a letter of referral, submit it to the Reception for First Time Visitors (#1).
Please complete reception procedures via the automated reception machines for returning patients.
Go to the department where you want to be examined, and wait to be called with your registration slip and Hospital ID in hand. If you are going to be examined at several departments, please wait at the first department (you can be examined at up to 3 departments in a day)

Notice: Patients without a letter of referral are required to pay a Special Medical Care Fee of 2,160 yen.

Returning Patients with Appointment

Please confirm your arrival by using the automated reception machines for returning patients. If you want to receive treatment at additional departments, complete the reception procedures needed. (Notice: after 11:30 am patients cannot add departments to be examined other than the one with which they have an appointment.) If you want to cancel or change your appointment, please contact us no later than the day before the day of your appointment.

Notice: Department of Neurosurgery, Mondays only Department of Orthopedic Surgery and Otorhinolaryngology are open only with a letter of referral or an appointment.

Prescriptions

Prescriptions are to be filled at outside pharmacies.
After seeing a doctor, you will receive a plastic file. Please hand it to the Desk for Cashier (#5).
After your payment, we will hand you the prescription at desk #6.
Please take the prescription to your family pharmacy and purchase the prescribed medicine.
If you need information on which pharmacies to go, see the receptionist for details.

Facsimile Transmission of Prescriptions

Prescriptions may be transmitted by facsimile to a family pharmacy for free from the lobby. (9:00 - 17:00)

General Information

At the General Information Counter, nurses give advice on consultation. Please feel free to ask questions.

Medical Consultation

The Regional Medical Center provides a consultation service for patients and their family in cooperation with experts.

  • on the use of health system and welfare services
  • on home care
  • on discharge and changing hospital
  • on health insurance and disability pension
  • on physical disability certificates and specified disease treatments

ER

Patients can be examined in the ER within consultation hours on weekdays. On weekends and after consultation hours on weekdays, the departments of internal medicine, pediatrics, and surgery are available.
If a doctor on duty is engaged in emergency surgery or handling a serious case, you may not be able to see him/her. Contacting us in advance (072-728-2001) is strongly recommended.
Make sure to bring your health insurance card and hospital ID card (if you are a returning patient) with you.
The ER gives first aid. Please consult again if so advised by the doctor, or if there is no improvement.

Forms and Certificates

  • Various forms and certificates are issued at the Desk for Admitting, Medical Certifications (#3)
  • Weekdays 9:00 - 17:00
  • Generally it takes about two weeks to issue
  • In principle, a patient needs to apply in person, but in case an agent is employed, a letter of attorney is required
  • Fill in the designated form before applying
  • Inpatients also can apply at Desk #3. If applied for in advance, you might receive documents required by the day of discharge
